How Do I Find Out If I Am Eligible for Medicaid Waiver Home Care Services?

If you or a loved one needs help with daily activities such as bathing, dressing, meal preparation, mobility, transportation, or other personal care needs, you may qualify for Medicaid-funded home care services.

Velora Home Care does not determine eligibility for Medicaid or waiver programs. Medicaid eligibility and enrollment decisions are made by the appropriate state agencies and program administrators. Once approved, we can provide services through a Medicaid Waiver program. If you are not approved for Medicaid Waiver services, we can still provide services through our private pay program.

Eligible for Medicaid Waiver Services

Once you are eligible, we can provide services through a Medicaid Waiver program.

We need:

• Your Medicaid identification number

• The name of your health plan (AmeriHealth Caritas, UPMC Health Plan, or PA Health & Wellness)

Once enrolled, you may have the right to choose your home care provider. If you choose Velora Home Care, we would be honored to serve as your trusted care partner.
